29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a) 1st floor, left side of building - Employees engaged in framing operations were exposed to fall hazards through unprotected floor holes in the walking/working surface at heights greater than six (6) feet above the grade without a fall protection system. b) 1st floor, back of building - Employees engaged in framing operations were exposed to fall hazards through unprotected floor holes in the walking/working surface at heights greater than six (6) feet above the grade without a fall protection system. Observed on or about 5/12/2016. A One Carpentry, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(4)(ii) which was contained in OSHA inspection number 1114211 citation number 1, item number 2 and was affirmed as a final order on 3/9/2016, with respect to a workplace located at1205 South 16th Street, Philadelphia, PA 19146.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not marked in a manner that readily identified them as defective, tagged with "Do Not Use" or similar language, or withdrawn from service until repaired. a) Jobsite - Portable ladder used to access the roof from the 4th floor by employee(s) had a bent/ damaged rung, exposing employees to fall hazards. Observed on 5/12/2016. A One Carpentry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(16) which was contained in OSHA inspection number 1114211 citation number 1, item number 4 and was affirmed as a final order on 3/9/2016, with respect to a workplace located at1205 South 16th Street, Philadelphia, PA 19146.
