29 CFR 1910.23(a)(9): Every floor hole into which persons cannot accidentally walk (on account of fixed machinery, equipment, or walls) were not protected by a cover that leaves no openings more than one inch wide:  a.  The employer did not ensure that a hole in the floor near the MV-02 line Metering Capstan machine on the 4th floor was not guarded to protect an employee from stepping in and through the hole, exposing employees to trip and fall hazards.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a.  On or about May 11, 2016, at Gulf Cable, LLC., the employer did not develop and utilize energy control procedures for their employees while they were required to perform maintenance, preventive maintenance, and make adjustments on the MV-1 line extruder machines.  Employees are exposed to caught-in, crushing, amputations and electrocution.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a.  On or about May 11, 2016 at Gulf Cable, LLC, the employer did not conduct periodic inspections at least annually on lockout/tagout (LOTO) procedures and requirements.  Employees were not locking out machines and did not have record of periodic inspections for the machines they operated.  Employees are exposed to pinching, crushing, amputations and death.

On or about May 11, 2016, throughout the plant and warehouse areas, employees were operating forklifts daily without examining the conditions of the forklift before use, exposing employees to struck-by and crushing hazards.
