29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: (a) On or about May 11, 2016 and at times prior thereto employees were exposed to electrical hazards when internal wiring was exposed on an extension cord directly behind the plug, where the cord was being used by employees to provide power to portable lights. NOTE- Abatement certification and documentation are required for this item.

29 CFR 1926.405(j)(1)(i): Fixtures, lampholders, lamps, rosettes, or receptacles had live parts normally exposed to employee contact: (a) On or about May 11, 2016 and at times prior thereto employees were exposed to electrical hazards when internal wiring was exposed on outlets and a light switch which had no cover plates, while power was applied, where drywall employees were applying drywall compound (mud) next to the outlets, and a light switch with hand held tools. NOTE- Abatement certification and documentation are required for this item.
