29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. a) Federal Bureau of Investigations Academy, 1 Range Rd, Quantico, VA 22135: On May 9, 2016, at approximately 8:45 a.m., an FBI agent suffered a leg fracture while running a sprint, which resulted in an in-patient hospitalization. The employer did not report the in-patient hospitalization to the Occupational Safety & Health Administration until May 11, 2016 at approximately 4:00 p.m.. No abatement documentation or certification required.
