29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   On or about May 12, 2016, employees were not protected from a fall while removing rafters and roofing material from a commercial building, exposing employees to falls up to 13.6 feet.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  On or about May 12, 2016, employees working at heights up to 13.6 feet were not provided with training to recognize or minimize the hazards associated with a fall.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.   On or about May 12, 2016, employees used an extension ladder that did not extend at least three feet past the upper landing surface of the roof they were accessing, exposing employees to falls between 8.9 and 13.6 feet.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
