29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:     a) During the months of April and May, 2016, employees performed servicing and maintenance tasks when performing setups on CNC Mills and CNC lathes.  Employees were not required to control hazardous energy sources in the following sequence:       1) turn off and shut down the machines,     2) physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s),   3) affix lockout or tagout devices to each energy isolating device,     4) verify that isolation and deenergization of the machines or equipment have been accomplished.       Employees were exposed to finger laceration and crush hazards from unexpected start up and/or energy release of hazardous energy.  Alternative protection, such as machine guarding, was not adequate.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1904.19(d).

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.     1) During the months of April and May, 2016, the following machines were not adequately guarded to prevent access to the point of operation during operating cycles, exposing employees to amputation and laceration hazards:       (a)  Lathe #2 (Haas SN3079177).  There was no safety interlock on the machine door to prevent it from being opened during operation.     (b)  Lathe #1 (Mazak SN139375).  There was no safety interlock on the machine door to prevent it from being opened during operation.     (c) Lathe #3 (Puma 240).  There was no safety interlock on the machine door to prevent it from being opened during operation.  The interlock switch had been removed from the door and was laying on top of the machine.       (d) Lathe #5 (Miyamo SN BE30153S).  There was no safety interlock on the machine door to prevent it from being opened during operation.     (e) Mill #3 (Haas SN1083967).  There was no safety interlock on the machine door to prevent it from being opened during operation.     (f)  Mill #2 (Haas SN1083597) There was no safety interlock on the machine door to prevent it from being opened during operation.     Abatement documentation is required for instances (a) through (d) in accordance with the requirements of 29 CFR 1903.19(d).  Instances (e) and (f) were Corrected During Inspection.
