29 CFR 1926.1101(h)(2)(i): The employer failed to implement a respiratory protection program in accordance with 29 CFR 1910.134(b) through (d) [except (d)(1)(iii)], and (f) through (m), which covers each employee required by 29 CFR 1926.1101 to use a respirator: a) In the demolition and abatement project at International Paper Bldg #10, on or about 5-12-2016, the employer failed to ensure that respirators were properly cleaned as required by 29 CFR 1910.134(h)(1), in that the inside of the tight-fitting respirators were contaminated with white particulate matter, exposing the employees to a health hazard.
