29 CFR 1910.213(r)(4): Suitable guard(s) were not provided to reduce to a minimum the hazard due to the point of operation of the DeWalt radial arm saw:  a) On or about 05/03/2016 - Mill, DeWalt radial arm saw model #GP, serial #71578, an employee was injured while using the saw to cut a scrap piece of wood.  OR, "IN THE ALTERNATIVE"  29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about 05/03/2016 - Mill, DeWalt radial arm saw model #GP, serial #71578, an employee was injured while using the saw to cut a scrap piece of wood.
