29 CFR 1926.405(a)(2)(ii)(I): Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points: a) Plattsburgh State College, Walk-over project, on May 11, 2016, and at times prior, an extension cord was run through a doorway with the doorway closed, and no protection provided for the cord, exposing the cord to damage and employees to potential for electrical shock.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Plattsburgh State College, Walk-over project, on May 11, 2016, and at times prior, a 48 inch opening in the guardrail system existed, no other form of fall protection was in use, employees exposed to a 20 foot fall.
