On or about May, 2016, on the SW side of a newly built multi-family dwelling, two employees were exposed to a fall hazard when working greater than 10 feet from a wooden trash hopper that was elevated by a Genie rough terrain forklift without fall protection.

On or about May 11, 2016 and times prior thereto, at or near 18515 Bridgeland Creek Pkwy, employers installing trusses were exposed to a fall hazard when performing work from a wooden trash hopper that was elevated by a Genie rough terrain forklift without being trained by a qualified person to recognize hazards associated with scaffold use.
