Lakeshore Utility Trailer, Inc. located at 3235 Moline-Martin Rd. Millbury, OH; On or about May 6, 2016, the employer did not ensure modifications such as a hole in the forks was performed with manufacturers approval. Employees are required to use forklift to pull slider boxes, and are exposed to hazards associated with overloading a forklift and loss of stability.

Millbury, OH 43447; On May 6, 2016, the employer did not ensure employees handled stable and safely arranged loads. An employee was hospitalized when the slider box came loose from the ball hitch that was attached to the forks of the Hyster sit down LP forklift, pinning him between the slider box and the building.
