29 CFR 1910.146(d)(3): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations, including, but not limited to items (3)(i) through (3)(vi) of this paragraph: a) Brew House: The employer did not implement the procedures and practices necessary for the safe permit entry of the B cellar fermentation tanks by properly purging and ventilating the permit spaces and verifying that conditions in the permit spaces, including slip hazards, was acceptable throughout the duration of the authorized entry, on or about May 13, 2016. b) Brew House: The employer did not implement the procedures and practices necessary for the safe permit entry of the lauter tun by properly isolating the space through LOTO of all energy sources including, but not limited to, mechanical, steam, hot water, caustic feed and malt entry and verifying that conditions in the permit space was acceptable throughout the duration of the authorized entry, on or about May 13, 2016. Abatement Certification Required.
