29 CFR 1910.147(c)(6)(ii): The periodic inspection certification of energy control procedures did not contain the required documentation on identification of the machine or equipment, inspection date, employees included, and the person performing the inspection: On or about May 11, 2016, at Bourque Printing/DBA Champion Graphic Communication, Inc., located in Baton Rouge, LA the employer failed to certify that a periodic inspection of the energy control procedures were performed on the following equipment: 1) Polar Mohr Model 115A 45" Paper Cutter S/N 763-2130 2) Ryobi Limited/Itek Graphic Model 3985 Sheet Fed Offset Printing Press, S/N 3676ZV04246039.
