29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about May 13, 2016, employees were exposed to being struck by falling materials.  The employee on the ground was working under employees working on the 2nd floor without wearing a protective helmet.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On or about May 13, 2016, employees were exposed to a fall of approximately 20 feet.  The employees were performing framing activities on a newly constructed residential home. They were walking the exterior walls while setting trusses without wearing fall protection.
