29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection.    (a)  Yates village building G, during roof tear-off and repair:  On or about 5/11/16, extension cord used to power a porter-cable pancake compressor was not used with GFCI protection, exposing employees to a potential shock hazard.  Cord was connected to a 120V receptacle inside the mechanical room of the building.    (b) Yates village building G, during roof tear-off and repair:  On or about 5/11/16, extension cord used to power a circular saw on the roof deck was not used with GFCI protection, exposing employees to a potential shock hazard.  Cord was connected to a 120V receptacle inside a vacant apartment.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and/or were not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached:      (a) Yates village building G, during roof tear-off and repair:  On or about 5/11/16, anchor points for vertical lifelines were not secured with the required number of 12d nails which increases the probability of anchor point failure in a fall arrest or fall restraint event.   Employees were exposed to a fall hazard of 18 to 20' to ground level.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used.    (a) Yates village building G, during roof tear-off and repair:  On or about 5/11/16, employees using vertical lifelines and rope grabs for fall protection were not trained in the safe use of the Guardian fall protection equipment, including but not limited to:      -- Remaining connected to the lifeline at all times while on the roof deck     -- Proper installation of the lifeline anchor point with the required number of nails    -- Attaching only one snaphook to the anchor point at a time    -- Adjustment of the rope grab to keep excess slack out of the rope and work position to minimize swing falls      Employees were exposed to a fall hazard of 18 to 20' to ground level.
