29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. On or about December 10, 2015 at the worksite located at or near 2700 E. 3rd Street, Hope, Arkansas, the employer failed to report an employee amputation to OSHA within 24 hours. This condition prevented an investigation from being accomplished in a timely manner.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws. On or about May 5, 2016 and at times prior thereto, employees of Southern Bakery were exposed to electrical hazards because the plug of a fan in the shipping department did not provide strain relief, exposing employees to electrical hazards.
