29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or a combination of warning line system and safety monitoring system. At job site located at 13600 110th Terrace, Lenexa, Kansas, employee engaged in commercial roofing activities on a low-sloped roof with unprotected sides and edges were exposed to serious fall hazards, in that employee was working outside of the Warning System, and employer did not employ the use of a Safety Monitor for worker at the edge of the roof. As a result, employees were exposed to fall hazards up to approximately 32 feet above the surface below.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. At a job site located at 13600 110th Terrace, Lenexa, Kansas, worker was exposed to serious fall hazards in that they had not been trained in fall hazard recognition and the proper installation or use of the equipment, procedures and systems designed to mitigate those hazards.

Copies of the OSHA 300 logs, and 300A summaries for 2013, 2014 and 2015, were not provided to OSHA Compliance Officers within four business hours of their request.
