At job site located at 13600 110th Terrace, Lenexa, Kansas, employee engaged in commercial roofing activities on a low-sloped roof with unprotected sides and edges were exposed to serious fall hazards, in that employee was working outside of the Warning System, and employer did not employ the use of a Safety Monitor for worker at the edge of the roof. As a result, employees were exposed to fall hazards up to approximately 32 feet above the surface below.
