29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards:     On or about May 13, 2016, the employer failed to provide standard guardrails and toeboards on the station transformers which are located in the main facility towards the rear by where the overhead gantry crane is positioned when not in use.  Employees are working on top of these transformers which range in heights from approximately 5-12 feet with no fall protection system in place.
