Employees were doing roofing work above this ground man.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   (a) Jobsite:  On or about May 13, 2016, employees were working on a steep roof and they were either improperly tied off or they were not tied off at all.  The fall hazard to the employees was about 25 feet to the ground.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:   (a) Jobsite:  On or about May 13, 2016, employees were walking on a low sloped roof and they were not protected from falling to the ground.  The fall hazard to the employees was about 15 feet to the ground.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employees knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill:   (a) Jobsite:  On or about May 13, 2016, the employees needed to be retrained in how to properly use fall protection when working from a roof.  When employees were tied off while on the steep roof they were improperly tied off.  The employees were tying off to a carabiner that the employees had attached to the front of their safety harnesses at a point below their belt buckle.  In addition, none of the employees were utilizing their leg straps to their safety harnesses when they were tied off.

29 CFR 1926.1053(b)(6): Unsecured ladder(s) were used on surfaces that were not stable and level:   (a) Jobsite:  On or about May 13, 2016, employees were using a 32 foot aluminum extension ladder that was set up on the lower 3 in 12 pitched roof and the ladder was not secured.  This ladder was on an angle to accommodate the sloped angel of the roof.
