29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a) Mira Pak Machine - on or about May 12, 2016, employees were exposed to machine hazards associated with moving parts and in-going nip points while they cleaned unguarded belts in the Mira Pak, Saturn model 14 manufactured by Allpac Consulting, LLC, but the employer had not established an energy control program consisting of energy control procedures, employee training, and periodic inspections to ensure unexpected energizing, startup, or release of stores energy, exposing employees to machine servicing hazards.

29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting:    a) Mira Pak Machine - on or about May 12, 2016, employees packaged fry powder with the use of a Mira Pak, Saturn model 14 packing machine which employees were exposed to a horizontal rotating shaft less than seven (7) feet from the floor, attached for the Dayton 2N915J motor and conveying system, but the shaft was not guarded and exposed employees to caught-in hazards.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven 7 feet or less above floors or platforms were not enclosed:    a) Mira Pak Machine - on or about May 12, 2016, employees packaged fry powder with the use of a Mira Pak, Saturn model 14 packing machine which employees were exposed to horizontal sprocket wheels and chain less than seven (7) feet from the floor, attached for the Dayton 2N915J motor and conveying system, but the chain and sprockets were not guarded and exposed employees to caught-in hazards.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):    a) Wig Dryer Assembly Area - on or about May 12, 2016, employees had used a COM/TREK II electronic digital radio, but the flexible cord attached to the plug was wrapped in electric tape and was not being visually inspected before use for external defects.
