29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  (a) General Construction Works dba GCW on or about May 13, 2016, at the address of 1544 E. Wooster St. Bowling Green, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards, such as falls during job site operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a)  General Construction Works dba GCW on or about May 13, 2016, at the address of 1544 E. Wooster St. Bowling Green, OH: The employer did not ensure employees were protected from falls while performing wood framing activities on top a commercial structure.  During this activity, employees were exposed to falls up to 32 ft. to the ground below.

Wooster St. Bowling Green, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing various wood framing operations from elevated surfaces.
