29 CFR 1926.651(i)(3): Sidewalks, pavements and appurtenant structure shall not be undermined unless a support system or another method of protection is provided to protect employees from the possible collapse of such structures. (a) Location - Intersection of 1776 Rd and River Rd, Andover, MA On or about May 12, 2016 and at times prior, employees were exposed to struck by hazards while replacing an existing water line in an excavation with an undercut and unsupported utility duct bank.

29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section. (a) Location - Intersection of 1776 Rd and River Rd, Andover, MA On or about May 12, 2016 and at times prior, employees were not protected from excavation cave-in or collapse while working in an excavation which had a depth of up to six feet ten inches.
