29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which more than 6 feet or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  (a)  A & R Roofing at 1930 Blake Street, Denver, CO 80203:  On and before May 11, 2016, the exposing employer did not ensure that each employee working on an elevated landing was protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Employees were unloading materials from a forklift onto the deck, when an employee fell approximately 15 feet to the pavement and was seriously injured.  Condition resulted in employee exposure to fall hazards  of approximately 15 feet.
