29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks.    On or about February 22, 2016, the employer used an unapproved, job-made, wooden trash box as an attachment on the rough terrain forklift. The box was unstable and was not adequately secured to prevent lifting from the forks.    Reference from ANSI B56.1-1969, Safety Standards for Powered Industrial Trucks.   Part II Section 4 Design and Construction.  420 Fork Extensions. Fork extensions or other attachments shall be suitably secured to prevent unintentional lifting or displacement on primary forks.  Part III Section 6 Operator Safety Rules and Practices.  605 Loading, Paragraph A: Handle only stable or safely arranged loads. D. When attachments are used, particular care should be taken in securing, manipulating, positioning, and transporting the load.
