Employees used the forklifts on a daily basis.

On or about May 12, 2016, at Grating Fasteners, LLC, the employer failed to provide safety data sheets (SDS) to employee(s) that were using hazardous chemicals such as, but not limited to TAP Magic Cutting Fluid. Employees used the TAP Magic Cutting Fluid on the CNC Machine at least once a month.
