29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: a) West wall of the Job Trailer: A relocatable power tap designed and listed to provide power to high concentrations of low powered loads such as computers, TVs, stereos, printers etc., was used to provide power to high powered loads such as a Haier microwave oven; a Frigidaire compact refrigerator; and a Mr. Coffee coffee maker. This relocatable power tap was also found to be series connected (daisy chained) into another relocatable power tap which is also a violation of its' listing.

29 CFR 1926.404(b)(2): Outlet devices had an ampere rating less than the load being served. a) West wall of the Job Trailer: Two daisy chained relocatable power taps used to provide power to a Haier microwave oven; a Frigidaire compact refrigerator; and a Mr. Coffee coffee maker and ten (10) Retivis battery chargers with a total of 23.5 amps, was a larger amperage rating than the power taps' 15 amps.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s). a) On or about 05-11-2016 at 1260 Snow Point Road, Charleston Naval Weapons Station, site inspections were not being performed on a frequent and regular basis ensuring safety and health hazards were identified and corrected in a timely manner. NOTE: "Competent Person" means one who is capable of identifying existing and predictable hazards in surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has authorization to take prompt corrective measures to eliminate them. ABATEMENT CERTIFICATION IS REQUIRED
