Site- An employee was not protected from crushing injuries where the employer failed to instruct each employee in the recognition and avoidance of unsafe conditions associated with employees working on foot around heavy construction equipment and remaining visible to heavy equipment operators. On or about May 16, 2016 an employee was pinned under the tire of a John Deere 672G motor grader, sustaining severe crushing injuries, where the employer did not train the employee in the safest method carrying out their job responsibilities to prevent and minimize exposure to struck by hazards.
