29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about June 2, 2016, the employer failed to ensure the established procedure for the control of hazardous energy was utilized when employees were engaged in servicing the Callen #47 die cast machine.  An employee was exposed to the hazards of moving parts and thermal energy during servicing and adjustment of the ladle on the Callen #47 die cast machine.       b) On or about June 2, 2016, the employer failed to ensure the established procedure for the control of hazardous energy was utilized when employees were engaged in servicing the Callen #47 die cast machine.  Employees were exposed to the hazards of moving parts and in-going nip points during servicing of the visitrack system on the Callen #47 die cast machine.  c) On or about June 2, 2016, the employer failed to ensure a procedure for changing the plunger tip of a die casting machine was developed, documented, and utilized to control potential hazardous energy when employees changed the tip.  An employee sustained an amputation during servicing of the plunger tip on the Callen #47 die cast machine when the shot with the plunger tip was actuated with the employees hand on the tip.   In accordance with 29CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:     a)  On or about June 2, 2016, Callen Manufacturing Corporation did not conduct periodic inspections of the energy control procedures in the past 12 months to ensure that procedures and requirements of the standard were being followed.      In accordance with 29CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department or other group.     a) On or about June 2, 2016, the employer failed to ensure a procedure was utilized that afforded a group of two employees a level of protection equivalent to that provided by the implementation of a personal lockout device for the control of hazardous energy when the employees were engaged in servicing the visitrack system of the Callen #47 die cast machine, exposing the employees to the hazards of moving parts and in-going nip points.    In accordance with 29CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.
