29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:  WORKSITE: The Gradall, Model 534D-6 rough terrain forklift, Serial Number 0688561, which was not designed by the manufacturer to support scaffold platform was used to elevate and support a scaffold platform from which an employee was framing a canopy roof.

29 CFR 1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:  WORKSITE: The supported scaffold platform, that was supported by the forks of the Gradall, Model 534D-6 rough terrain forklift, Serial Number 0688561 for an employee framing a canopy roof was not positively secured.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   WORKSITE: The employee, framing a canopy roof from a supported scaffold platform was not protected from the hazard of falling approximately 20 feet to the ground by the use of an appropriate fall arrest system.
