29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: (a) On or about 5/12/2016 and at times prior employees were exposed to a 20 foot fall hazard while drilling a five inch core through a building for a vent line from an articulating boom without the benefit of fall protection in the vicinity of 200 West Main Avenue in Bismarck, North Dakota.
