29 CFR 1926.451(g)(1)(i): Each employee on a  ladder jack scaffold was not protected by a personal fall arrest system.  a) 1308 Meadowbrook Drive, Syracuse, NY, 13212, On or about 5/17/16: The employer did not provide fall protection on a ladder jack scaffold, that had a fall distance of 10 feet 2 inches to the ground below.  The American Roofing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1926.451(g)(1) which was contained in OSHA inspection number 910761, citation number 1, item number 1 and was affirmed as a final order on 7/26/13 with respect to a workplace located at 139 Rigi Avenue, Syracuse, NY 13206.
