The employer has not provided comprehensive respirator training to all employee who are required/provided respirators.  The employer was unable to ensure that each respirator user could demonstrate knowledge of the procedures for respraitor maintenance and storage: respirators were observed stored hangings on hooks at work stations. Respirators were observed hung on hooks in the work area , not cleaned or stored correctly; some employees wearing respirators wore full beards; medical evaluations were conducted after respirators were issued and required to be worn; not all employees were provided respirators at the onset of employment; employees did not receive the required training to wear respirators prior to issue; employees were not aware that the company has a written respiratory protection program. The employer has not conducted evaluations of the workplace to ensure  the written respiratory protection program was being effectively implemented.  ABATEMENT CERTIFICATION REQUIRED
