Barre Rd. Archbold, OH: The employer did not ensure that machine specific energy control procedures were utilized to control the energization of the #8166 Litho-Laminator Machine during cleaning task(s). Employees performing cleaning tasks near the input roller and drum in an energized condition were exposed to nip points between the machines combo roller and cylinder drum.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) Archbold Container Corp. on or about May 12, 2016, at the site located at 800 W. Barre Rd. Archbold, OH: The employer did not ensure that energy isolating procedures for shutdown were performed prior to performing cleaning tasks on the #8166 Litho-Laminator Machine. Employees performing these task in energized condition were exposed to amputation hazards at or near the machines point of operation (between roller nip-points), in that energy control shutdown procedures required under 29 CRF 1910.147(d)(2) were not utilized before the employee began the aforementioned work task on the machine.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a) Archbold Container Corp. on or about May 12, 2016, at the site located at 800 W. Barre Rd. Archbold, OH: The employer did not ensure that employees who are exposed to energized rotating parts during service and cleaning work on the #8166 Litho-Laminator were adequately trained in the skills required for the safe application, usage, methods, and means to isolate and control potential hazardous energy sources. The employees tasked with cleaning activities on the aforementioned machine were not specifically trained to lockout the machine to protect against amputation injuries prior to performing cleaning work at the point of rotation (nip-point).
