29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a)  Archbold Container Corp. on or about May 12, 2016, at the site located at 800 W. Barre Rd. Archbold, OH: The employer did not ensure that machine specific energy control procedures were utilized to control the energization of the #8166 Litho-Laminator Machine during cleaning task(s). Employees performing cleaning tasks near the input roller and drum in an energized condition were exposed to nip points between the machines combo roller and cylinder drum.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:    a) Archbold Container Corp. on or about May 12, 2016, at the site located at 800 W. Barre Rd. Archbold, OH: The employer did not ensure that employees who are exposed to energized rotating parts during service and cleaning work on the #8166 Litho-Laminator were adequately trained in the skills required for the safe application, usage, methods, and means to isolate and control potential hazardous energy sources. The employees tasked with cleaning activities on the aforementioned machine were not specifically trained to lockout the machine to protect against amputation injuries prior to performing cleaning work at the point of rotation (nip-point).
