29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a) 420 Willard Avenue Job Site - On or about 5-14-16 - Employee was exposed to rotating parts of a Toro MX5400 E-Z Vac bagger and blower attachment that was not guarded to prevent access to rotating parts before they stopped spinning.
