29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:   (a) Production Floor - On or about May 24, 2016, machine operators, engaged in set up of the Seamers, to include cleaning the rollers and blade changes, were not trained as authorized employees in that they did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.    (b) Production Floor - On or about May 24, 2016, all machine operators had not received training at the affected level in that they had not been instructed in the purpose and use of the energy control procedure.    (c)  Production floor - On or about May 24, 2016, all other employees whose work operations are or may be in an area where energy control procedures may be utilized, shall be instructed about the procedure, and about the prohibition relating to attempts to restart or reenergize machines or equipment which are locked out or tagged out.

(b) Production Floor - Seaming Area - On or about May 24, 2016, an employee was exposed to hazards created by the blades on the layon roller in that the guard interlock device was inoperable.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees, in that the employer did not ensure the safety of the equipment pursuant to the considerations in sections (i) through (viii) of 29 CR 1910.303(b)(1):  (a) Production Area - On or about May 24, 2016, the electrical cabinet on the SL21 Sleeve Machine operating at 240V was not free from recognized hazards in that the disconnect switch was disconnected from the internal parts creating electrical hazards for employees de-energizing the equipment.
