Bakery - Packing area: The Le-matic modular roll slicing machine model # RS-S, Serial #919188 had two unguarded chains and sprockets and one unguarded belt and pulleys located approximately 2.5 feet from the floor, the unguarded chains, sprockets, belt and pulleys exposed machine operators to amputations, occurred on or about 05/04/2016.    d) Baker Boys LLC DBA Formica Bros. Bakery - Winkler machine/bread line area: The Winkler machine also known as hamburger bun maker had approximately 8 guards missing that covered the chains and sprockets located within 1 to 8 feet from the floor, the unguarded chains and sprockets exposed machine operators to amputations, occurred on or about 05/04/2016.    e) Baker Boys LLC DBA Formica Bros.
