On or about May 18, 2016, the employer did not ensure that employees who were required to wear filtering face piece respirators were medically evaluated prior to wearing the respirators. Employees working in the production area using metal dusts were required to wear 3M P100 filtering face piece without the benefit of a medical evaluation.

Wipe sampling indicated the contamination of 141.10 ug/100cm2 of lead, approximately 7 times the HUD guidelines for surfaces on the top of the respirator storage container.
