29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910:  (a) Main Building Lighting Transformer Area - On or about April 14, 2016, an electrical arc flash incident took place when the cover was being removed from the Olsen Electrics Corporation 330 kVA/460 VAC floor-mounted transformer and the cover fell into the energized conductors of the transformer.  A wall had been installed in the transformer area that partially blocked access to the right front side of the transformer and the cover's associated attachment hardware, not allowing a full 3 feet of working space for the entire width of the transformer.  (b) Main Building - Welding shop, access to the new Hammond Power Solutions 15 kVA/460 VAC transformer mounted to a structural column was blocked by the Colchester lathe, not allowing a full 3 feet of working space in front of the transformer.  (c) Main Building - Welding shop, access to two of three circuit breaker panels adjacent to the Hammond Power Solutions 15 kVA/460 VAC transformer was partially blocked by a metal 4-drawer cabinet and a pedestal-mounted grinder, not allowing a full 3 feet of working space in front of the circuit breaker panels.

29 CFR 1910.303(g)(1)(i)(B): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the width of the working space in front of electrical equipment was less than the width of the equipment or 762 mm (30 inches), whichever was greater :  (a)  Main building lighting transformer area - On or about April 14, 2016, an electrical arc flash incident took place when the cover was being removed from the Olsen Electrics Corporation 330 kVA/460 VAC floor-mounted transformer (48 inches wide) and the cover fell into the energized conductors of the transformer.  A wall had been installed in the transformer area that partially blocked access to the right front side of the transformer and the cover's associated attachment hardware, not allowing a 48-inch wide working space in front of the transformer.

29 CFR 1910.303(g)(1)(i)(C): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the width of work space was not clear and did not extend from the grade, floor, or platform to the height required by paragraph (g)(1)(vi) of 29 CFR 1910.303:  a) Main building lighting transformer area - On or about April 14, 2016, an electrical arc flash incident took place when the cover was being removed from the Olsen Electrics Corporation 330 kVA/460 VAC floor-mounted transformer and the cover fell into the energized conductors of the transformer.  A wall had been installed in the transformer area that partially blocked access to the right top side of the transformer, not allowing a full 6 foot 6 inch headroom work space for the entire width of the transformer.

29 CFR 1910.253(b)(5)(ii)(G): Cylinder valves shall be closed when work is finished.  (a) Main Building Welding Shop - Oxy-Acetylene cutting/welding torch on a cart in the center of the shop was not in use and the oxygen tank had not been closed at the completion of its last use. Pressure on the regulators indicated over 1000 PSI on the inlet side and 80 PSI on the outlet side.
