29 CFR 1926.502(d)(21): Personal fall arrest systems shall be inspected prior to each use for wear, damage and other deterioration, and defective components shall be removed from service. a. On or about May 18, 2016 - at a jobsite located at 315 Lake Street, North Aurora, Illinois, 60542, Employees were removing / replacing roofing material on a residential building. The employer failed to remove defective / damaged personal fall arrest systems from use. Employees were thereby exposed to fall hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards: a. On or about May 18, 2016, at a jobsite located at 315 Lake Street, North Aurora, Illinois, 60542, employees were working from an elevation greater that 6 feet. The employer failed to ensure an employee training program specific to fall hazards had been developed and maintained. Employees were thereby exposed to the hazards associated with falls from height. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a. On or about May 18, 2016, at a worksite located at 315 Lake Street, North Aurora, Illinois, 60542, employees of ABC Roofing and Siding, Inc. were working on a residential roof and were not using fall protection. Employees were exposed to a fall hazard of approximately 14 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d). The ABC Roofing & Siding Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 562518, citation number 1, item number 1 and was affirmed as a final order on Wednesday, September 26, 2012, with respect to a workplace located at 307 Heustis, Yorkville Illinois.
