29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.  a. On May 18, 2016, employees were nailing shingles to the roof with a pneumatic nail gun without eye protection, thereby exposing employees to a struck by hazard.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502.  a. On May 18, 2016, employees were observed working on the roof of a multilevel single family home without fall protection, thereby exposing the employees to a fall hazard of greater than 15 feet.

29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided.  a. On May 18, 2016, the employees were climbing onto the second story roof from the porch roof which had a break in elevation of greater than 19 inches, thereby exposing employees to a fall hazard of 13 feet.

29 CFR 1926.502(d)(17): The attachment point of the body belt shall be located in the center of the wearer's back. The attachment point of the body harness shall be located in the center of the wearer's back near shoulder level, or above the wearer's head.  a. On May 18, 2016, the employer was working on a 10x12 pitch roof over 15 feet while using a rock climbing harness which does not attach on the wearers back, thereby exposing the employee to a 15 foot fall hazard.
