Employees operated the dishwasher which uses chemicals and employees change the dishwasher chemicals. This condition exposed employees to chemical hazards associated with the use of chemicals such as, but not limited to:  1. Clean Force Dishmachine Detergent Low Temperature; 2. Clean Force Rinse Additive for High Solids; 3. Low Temp Dishmachine Sanitizer; 4. Clean Force Pot & Pan Detergent Super Concentrated; 5. Clean Force Enzyme Presoak; 6. 22 Multi-Quat Sanitizer; 7. Clean Force Pot & Pan Detergent.

Employees perform duties in the kitchen to support restaurant operations.  The employer has implemented work rules for the use of PPE; however a workplace assessment has not been performed and certified by the employer. Employees performed operations where they were exposed to chemical and slip hazards, including but not limited to the following:  1. Exposure to chemical hazards when dishwashing; 2. Exposure to slips, trips and falls while working in the kitchen.
