On or about May 19, 2016, an employee was exposed to a trench cave-in in at 583 Tree side, Stow, Ohio 44224. The employee was performing water line replacement in an approximately 7 foot deep trench with accumulated water on the bottom.

On or about May 19, 2016 an employee was exposed to a trench cave in in at 583 Tree side, Stow, Ohio 44224. The employee was performing water line replacement in an approximately 7 foot deep trench without any form of cave in protection.

The employee was performing water line replacement in an approximately 7 foot deep trench, the ladder used to access the trench did not extend 3 feet over its landing area.
