OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the load should the components of the overhead hoist/bridge cranes fail:    a)  Main Bay, on or about 5/19/16:  Safety latch was broken on the lifting hook for the CM 3 ton underhung bridge crane MBH#1.    b)  Paint Shop, on or about 5/19/16:  Safety latch was broken on the lifting hook for the CM 3 ton underhung bridge crane PSH#2.    c)  Main Bay, on or about 5/19/16:  Upper limit switch on a CM 3 ton underhung bridge crane MBH#3 was not functioning.    d)  Plate Shop, near Atlantic shear, on or about 5/19/16:  Safety latch was broken on a lifting hook for the Coffing 2 ton overhead hoist.      Abatement certification must be submitted for these items.

29 CFR 1910.24(h): Standard railings were not provided on the open sides of all exposed stairways and stair platforms:    a)  Angle Bay, on or about 5/19/16:  A fixed stairway and platform with 5 risers to the storage rack next to the Peddinghaus Angle Master was not adequately guarded in that there were no standard railings on the end of the platform, no stairway stair railings on the left side descending, no midrails on the platform and no midrails on the stairway on the right side descending.
