29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by: The employer is failing to protect employees from amputation hazards associated with operating inadequately guarded machines. This was most occurrence of this was evident during the inspection of the facility located at 4432 McKinnley St. Omaha, NE 68112, where the employer has employees using the C109 Automatic Welding Technology auto welder to weld split parts by inserting the parts into the area where the hydraulic clamps that hold the part for welding are actuated by a single action.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2): The employer is failing to protect the employees from amputation hazards associated with operating machinery without having conducted a written hazard assessment. The most recent occurrence of this was evident in the inspection of the facility located at 4432 McKinnley St. Omaha, NE 68112, where the employer has an employee operating C109 an automatic welder while wearing mandatory PPE and using informal work procedures. Instances include but are not limited to: i. Employees are required to wear welding gloves while operating the automatic welder without the employer having conducted a written hazard assessment that outlines the potential hazards, abatement methods and necessary PPE. ii. Employees operating the automatic welder without a standardized written work procedure, a normal product of conducting a written hazard assessment that outlines the potential hazards, abatement methods and necessary PPE.
