Instances include but are not limited to:       i.  Employees are required to wear welding gloves while operating the automatic welder without the employer having conducted a written hazard assessment that outlines the potential hazards, abatement methods and necessary PPE.    ii.  Employees operating the automatic welder without a standardized written work procedure, a normal product of conducting a written hazard assessment that outlines the potential hazards, abatement methods and necessary PPE.
