On or about May 18, 2016 and at times prior thereto employees were exposed to struck by hazards from potential flying wood, plastic and metal  debris by use of cutting equipment without safety eye wear.

On or about May 18, 2016 and at times there to employees were exposed to shock hazards when working around energized equipment by not having each work area room's disconnect means clearly indicated on the electrical panel box breaker panel switches.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):   On or about May 18, 2016 and at times prior thereto employees were exposed to electrical shock hazards when working on or near a power outlet located in the back of shop cutting room that did not have a cover.
