Note: The elements of an effective safety and health program may include, but are not limited to, the following: a) Management commitment and leadership b) Assignment of Responsibility c) Identification and control of hazards d) Training and education e) Recordkeeping and hazard analysis f) First aid, medical training and assistance g) Frequent and regular inspections  a) Jobsite, 156 La Belle Street, Pittsburgh, PA:  On or about May 13, 2016, employees are exposed to hazardous conditions and the employer did not have a safety and health program that would have included, but not limited to, identifying and controlling hazards, training and education and performing inspections.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii):  a) Jobsite, 156 La Belle Street, Pittsburgh, PA:  On or about May 13, 2016, four employees are installing shingles on a residential roof and are not utilizing fall protection.  The fall hazard to the employees is approximately 35-40 feet to the ground.  The employees are not trained by a competent person that is qualified to meet the requirements, on how to recognize and protect themselves from the dangers of falling.

The fall hazard to the employees is approximately 25 feet to the ground.
