29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. a. On or about May 19, 2016, at the above addressed jobsite, employees were performing grinding operations from a tubular welded frame scaffold located approximately twenty seven (27) feet above the ground. The employees were not provided with a guardrail system or personal fall arrest system, thereby exposing the employees to fall hazards.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as used as a means of access. a. On or about May 19, 2016, at the above addressed jobsite, an employee conducting grinding operations from a tubular welded scaffold, approximately twenty seven (27) feet above the ground, were not provided with an access ladder, thereby exposing the employee to falls.

On or about May 19, 2016, at the above addressed jobsite, an employee was conducting tuck pointing operations while standing on the mid rail of the aerial lift. The employee was exposed to fall hazards.
