29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    a) At the jobsite, on or about May 19, 2016, employees were performing wood framing activities that included operating portable electric hand tools such as pneumatic nail guns, and circular saws without the use of eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) At the jobsite, on or about May 19, 2016, employees working on a second working level while performing wood framing were exposed to open floor joists and exposed leading edges without fall protection provided, exposing employees to fall hazards of up to 19.5 feet onto the ground (outside the structure) or concrete floor (inside the structure).      b) At the jobsite, on or about May 19, 2016, employees were working near and/or getting materials through a window opening on the second floor without any fall protection system in place, thus exposing employees to a fall hazard of 11 feet onto the ground.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     a) At the jobsite, on or about May 19, 2016, employees used an aluminum extension ladder that only extended about 2 feet above the upper landing surface of a second working level during wood framing activities of a new single family residential dwelling, resulting in fall hazards of up to 10.5 feet onto concrete.
