29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s).    Employees performing sanding operations by hand on CNC metal lathes were exposed to struck-by and caught in hazards.    Industrial Screw Conveyors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii) , which was contained in OSHA inspection number 943893, citation number 1, item number 2-b and was affirmed as a final order on April 28, 2014 with a final abatement date of July 30, 2014, with respect to a workplace located at 4133 Conveyor Drive, Burleson, Texas 76028.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.     The employer failed to report to OSHA a work-related hospitalization prior to the elapse of the 24 hour reporting period of the incident.
