On or about May 19, 2016, in the fabrication area where employees were exposed to struck-by and caught-by hazards when operating a Doosan PUMA 2000Y CNC machine without functioning interlocks. b. On or about May 19, 2016, in the fabrication area where employees were exposed to struck-by and caught-by hazards when operating an Doosan PUMA 200 CNC machine without functioning interlocks. c. On or about May 19, 2016, in the fabrication area where employees were exposed to struck-by and caught-by hazards when operating a Doosan PUMA 240 CNC machine without functioning interlocks. d. On or about May 19, 2016, in the fabrication area where employees were exposed to struck-by and caught-by hazards when operating a Doosan PUMA 280 CNC machine without functioning interlocks. e. On or about May 19, 2016, 2013, in the fabrication area where employees were exposed to struck-by and caught-by hazards when operating an Johnford ST40 turning center machine without functioning interlocks. f.
